1. What is Form A?

This form is related to the Individual Applications under subsection (2) of section 15 of the payment and wages act, 1936 by or on behalf of an employed individual shall be made in Form A of the payment of wages rules, as the case may be, one copy of which shall bear such court-fee as may be prescribed and shall be submitted in the court of authority.

2. Why is it Important?

This form A contains information regarding the parties, their claims, statements, relief etc. As per section 15 (2) were contrary to the provisions of this Act any deduction has been made from the wages of an employed person, or any payment of wages has been delayed, such person himself, or any legal practitioner or any official of a registered trade union authorised in writing to act on his behalf under this Act, or any other person acting with the permission of the authority appointed under sub-section (1), may apply to such authority for a direction under sub-section (3) in form A.
