1. What is Form IV Rule 21 (1)?

Form IV is related to Application for License to the licensing officer, every application by a contractor for the grant of license shall be made in form IV and every application shall be accompanied by the certificate by the Principal employer in form V to the effect that the applicant has been employed by him as a contractor in relation to the establishment. 

2. Why is it Important?

Form IV is important because it contains the particulars of the contractor, principal employer, their address, nature of work, duration of contract work, whether the contractor was convinced of any offence within the preceding five years etc. and shall be duly signed by the principal employer. Whereas form V is to be accompanied by form IV as it is a declaration or a certificate by the principal employer that he has engaged the contractor for the establishment as per all the provisions of Contract Labour ( Regulations & Abolition ) Central Rules 1971.
